REGISTRATION 2012

Online registration will be available Feb 12th-Feb 24th.  Walk in registration dates are Fri Feb 3rd, 5:30-8:00pm and Sat Feb 11th, 9:00am-1:00pm at the West Lorne Arena
 
In order to complete your childs registration we will require the following items:

A PHOTOCOPY of the childs birth certificate

A registration form filled out and signed

A Parents code of conduct signed

Payment

Without all of these items we will be unable to process your childs registration.

In order to complete your registration you will need to go to the forms section, print off the forms, complete forms and mail them in along with all the items listed above to complete your registration.  Please mail in everything to the following address:

Please note that all registrations must be received by FEBRUARY 27th, 2010

LATE REGISTRATIONS WILL ONLY BE CONSIDERED AFTER FEBRUARY 27TH  IF THERE IS STILL ROOM ON AN EXISTING TEAM IN YOUR CHILDS AGE GROUP.

A $50.00 LATE FEE WILL ALSO APPLY
